Case Management
Purpose:
A&S Unlimited Solutions uses case management as a collaborative process of assessments, planning, coordinating, advocacy and evaluation to meet family needs. During this one on one session clients will meet with their case manager to evaluate the needs and create a plan of action to address their identified needs.
